Passport Acceptance Agents in Toledo
If you are not travelling for 2-3 months then your best option is to visit your local Passport Acceptance Facility. Passport Acceptance Agents can be found at most post offices, county clerks, courthouses, and at some public libraries. Make sure you call the Passport Acceptance Facility before going to see if they require an appointment.
A Passport Acceptance Agent is used for new passports, first-time passports, child/minor passports, lost passports, and for applicants who are not eligible to renew their passports. A Passport Acceptance Agent an individual certified and approved by the U.S. State Department to review your passport application documents. The Passport Acceptance Clerk will then execute (approve) and seal them into an official envelope for processing by a regional passport agency.
Toledo Passport Acceptance Agent Requirements
When you go to have your passport application executed and sealed into an official envelope, make sure to bring all of these items:
- Proof of US citizenship must be the original or certified copy of your birth certificate or naturalization certificate.
- Identification – You will need valid identification, such as a state driver’s license, state ID or military ID that is at least six months old.
- Completed DS-11 passport application – do not sign your passport application until the Passport Acceptance Agent tells you to do so. We suggest filling out your application ahead of time, but you can also obtain blank DS-11 forms at the Passport Acceptance Facility.
- Passport style photo – you must submit a new 2x2 passport photo with a white background, wearing no uniform, no glasses, no headgear (i.e. - eye glasses, eye patches, hats or orthodontic apparatus).
- A check or money order – you will need to pay for the passport fee and the Passport Acceptance Agent service fee. The clerks normally does not accept cash.
IMPORTANT – If applying for a child's passport, if both parents names' are on the birth certificate, the child together with both parents must be present to execute the application. If for some reason one parent cannot be present, that parent can fill out a Parent Consent Form DS3053.
