Do You Need a New Passport (DS-11)?

Not sure which form you need? Here's a simple checklist. If any one of the conditions below applies to you, you must use Form DS-11 and apply in person at an acceptance facility. You Qualify for a New Passport (DS-11)

  • You are applying for a U.S. passport for the very first time
  • Your most recent U.S. passport was issued when you were under age 16
  • Your most recent U.S. passport was issued more than 15 years ago
  • Your most recent U.S. passport was lost, stolen, or damaged (water damage, torn pages, mutilated)
  • Your name has changed since your last passport was issued and you cannot legally document the name change
  • You are applying for a passport for a minor child under age 16
  • You are 16 or 17 years old applying for your first adult passport

Important: All DS-11 applications must be submitted in person at a passport acceptance facility or passport agency. This includes first-time applicants, children, and anyone replacing a lost, stolen, or damaged passport.

What You'll Need

New Passport Requirements & Documents

Here's a complete checklist of everything required for a new passport application (Form DS-11). If your previous passport was lost or stolen, you'll also need Form DS-64. We'll review all your documents before you go to your acceptance facility.

Proof of U.S. Citizenship

  • Certified U.S. birth certificate (original or certified copy — no photocopies)
  • Consular Report of Birth Abroad (Form FS-240)
  • Certificate of Naturalization or Certificate of Citizenship
  • All documents must be original or certified copies with a raised seal

Proof of Identity

  • Valid driver's license (not temporary or learner's permit)
  • Current government ID (city, state, or federal)
  • Current military ID (military and dependents)
  • Certificate of Naturalization may also serve as both proof of citizenship and identity

Passport Photo

  • One 2×2 inch color passport photo
  • Taken within the last 6 months
  • White or off-white background
  • Full face, front view with a neutral expression
  • No glasses (as of 2016 regulation)

Form DS-11 and DS-64 (if applicable)

  • Complete Form DS-11 online or by hand in black ink
  • Do NOT sign the form — you must sign it in front of an acceptance agent
  • Use your full legal name as it appears on your citizenship evidence
  • Provide your Social Security number (required by law)
  • If your previous passport was lost or stolen, also complete Form DS-64 (Statement Regarding Lost/Stolen Passport)
