Replacing a lost or stolen passport requires both Form DS-64 (to report the loss) and Form DS-11 (the new passport application). Here's everything you need to bring to your acceptance facility appointment.
Form DS-64 (Lost/Stolen Statement)
- Complete Form DS-64 — Statement Regarding a Lost or Stolen Passport
- Describe when, where, and how the passport was lost or stolen
- Once submitted, the lost passport is permanently invalidated — it cannot be used even if found later
- Submit DS-64 together with your DS-11 new passport application
- This is required by law — you cannot replace a lost/stolen passport without it
Proof of U.S. Citizenship
- Certified U.S. birth certificate (original or certified copy — no photocopies)
- Consular Report of Birth Abroad (Form FS-240) if applicable
- Certificate of Naturalization or Certificate of Citizenship
