Proof of Round-Trip Travel

  • Photocopy of round-trip airline ticket confirmation with your name
  • Photocopy of confirmed hotel reservation from a Chinese hotel
  • Both documents can come from a travel agent but must include all details

Valid Photo ID Copy

  • Color copy of both front and back of Driver's License, State ID, or Military ID
  • If your name has changed, provide marriage certificate or court order

Proof of Residence Form

  • Complete the Proof of Residence Form in black ink
